FAQ

Do I need an appointment?

Yes. All patients are seen on an appointment basis and scheduled within 24 to 48 hours of the initial call. We try to accommodate our patients’ schedules as best we can, however, staff availability or insurance verification may delay the process.

We require 24 hours cancellation notice.

What do I need to bring to my appointment?

For your initial evaluation please arrive 10-15 minutes prior to your scheduled appointment. In order for the paperwork to be completed we require:

  • Photo ID
  • Insurance Card
  • Prescription and/or Physician Referral
  • Film Reports (MRI/X-Ray)
  • List of Medications
  • Medicare Card
Will my insurance cover this?

Pro-Rehab Services will contact your insurance provider to verify therapy benefits prior to your initial evaluation. At this time we will notify you of co-pays and unmet deductibles. Keep in mind this is a verification of benefits, not a guarantee of payment. Therefore patient may be responsible for a portion of the services received from Pro-Rehab Services.

Do I need a referral from my doctor?

Certain insurance policies require an additional referral from your primary physician (PMD) in addition to an order for therapy.

It is your responsibility to bring the required prescription/referral at the time of your initial visit. A prescription or referral can be faxed to our office at (708) 489-6303. If you need us to contact your doctor for a referral, call us prior to your initial visit. We cannot treat you without a prescription or order.

Upon initial evaluation, your therapist will create a plan of care in discussion with you and will send it to your referral source for signature.

Do you send reports to my doctor?

At Pro-Rehab Services we believe in regular and ongoing communication with referral sources. After the initial evaluation, a report will be sent to your referral source along with a plan of care. Throughout your physical therapy care, periodic progress reports will be sent to the appropriate referral source. In addition, a progress report will be sent with the patient for follow-up appointments with your healthcare provider. We request that the patient informs us of upcoming appointments.

A final evaluation/discharge summary report will be sent to your referral source upon discharge from Pro-Rehab Services. Upon request, Pro-Rehab Services will send additional copies to your physician as required.

What is your policy for scheduling?

It is our policy to schedule new patients within 24-48 hours of the referral. In some cases, evaluations are scheduled later than 48 hours based on physician request, your availability, or staff availability.

Visit frequency is determined between you and your therapist based on medical need and physician orders. We schedule subsequent appointments for one week at a time. Should you wish to schedule more than one week at a time, talk to your therapist. Missed appointments are to be made up within the same week.

What are your hours?

We are open Monday through Friday from 8:00 a.m. to 6:00 p.m. Saturday appointments are available upon request.

Pro-Rehab Services will provide a customized home exercise program for each patient. It is our belief that a patient who thoroughly understands his or her condition will participate more in their recovery process to achieve the most favorable outcome. If at any time during the course of treatment you have questions or concerns, please speak directly to your therapist.

For your comfort, we ask that you wear loose clothing for your therapy session. Tight jeans and other tight-fitting clothes are not appropriate for therapy sessions. You may want to bring exercise shorts, loose-fitting clothes, and athletic shoes if you choose to change into when you come.
